Looks like mange. With the intense itching and scabs it could very well be sarcoptic mange which is contagious to humans. You are going to need a skin scraping at the vet to confirm if its mange and whether its demodex or sarcoptic.

You can get the Equate Multi-vitamins at Walmart. It's like 8 bucks for 300 pills. Just break in half and give 1/2 a pill a day.

It might not be his joints that are cracking. It is possible that what you are hearing is the tendons popping as they move over the joints or bone.
