You should get a second opinion. Your vet should give you options. We treat this with oral ivermectin as well and possibly an antibiotic. Treatment takes time, but works well. For the urinary problems I would start with a urine culture and/or ultrasound to rule out UTIs and/or stones or crystals. a urine sample will show this as well, common place is to obtain a sterile sample from you vet via cystocentesis.
