Maybe it's a contact allergy since it's on her back? Does she get a chance to roll in grass every so often? Do you use any cleaning agents in the carpets? Have you always used the same detergent for washing blankets/bedding?

To me, it actually looks like it may be an infection. If it spreads or worsens you would need abx.
