Ugh hair loss

Grendel has some hair loss just under his nose and it's red I haven't noticed any itching or rubbing at it... I'm worried about demodex again but the last two times the hair loss was on his neck, and this time it it's localized to his nose. Going to give it the weekend and take him in next week if it doesn't improve. Any ideas of what it could be beside mange?

yeah, its on both sides of his nose too... Damnit , its been almost a year and a half since he had it last time poor guy, he gets good food and immune suppliments too
